If you recently attended a public hospital Emergency Department (ED) or Urgent Care Centre for a broken bone, you were likely discharged with a temporary plaster backslab (a half-cast held in place by elastic bandages) and instructed to arrange a follow-up review within 5 to 10 days.

While an emergency backslab provides crucial early immobilisation while acute swelling is at its peak, it is not designed for long-term recovery. Follow-Up Fracture Care & Custom Casting involves transitioning from this temporary emergency splint to a custom-moulded, functional cast or brace once the initial swelling subsides.

Why Was a Temporary Backslab Applied in the Emergency Department?

During the first 48 to 72 hours after a bone injury, surrounding soft tissues swell significantly. If a rigid, fully enclosed circular cast is placed on a newly broken limb, this swelling has nowhere to expand, creating a dangerous build-up of internal pressure (compartment syndrome).

An open-sided plaster backslab accommodates this initial inflammation safely. However, once the swelling resolves, the backslab becomes loose, compromising bone stability and increasing the risk of bone displacement.

 

What Happens During a Cast Conversion Appointment?

At our outpatient clinics, your follow-up assessment follows a structured pathway:

  1. Clinical & Skin Assessment: The temporary backslab is carefully removed, and the clinical team inspects the skin for pressure areas, swelling reduction, and neurovascular function.
  2. Check Radiography Coordination: Follow-up X-rays are reviewed to confirm the fracture alignment remains anatomical as swelling drops.
  3. Definitive Casting or Splinting: Depending on the stability of your fracture, our plaster technicians apply a custom-fitted synthetic fibreglass cast, a breathable waterproof cast, or a removable thermoplastic splint.

All three of the following options are supplied and fitted on-site at both our Southport and Springwood (Logan) clinics:

⚠️ Important Emergency Red Flags:If you experience cold, pale, or blue fingers/toes, severe numbness, pins and needles that do not improve with elevation, or pain that worsens significantly despite medication, your cast may be too tight.
